Office Information

Forms and information to guide you through the process.

Scheduling Appointments
To schedule an appointment, please call during our office hours. We will ask you several questions to explain the nature of your visit, so that we may determine the best time for your appointment. Please have your insurance card available and mention to our scheduling associate if you prefer a specific location.

Appointment Cancellations and Lateness

If you must cancel or reschedule your appointment, please give us as much notice as possible as it will allow another patient to use your appointment time. If you’re running late for your appointment, as a courtesy, please contact us as well. It is Practice policy for the 20 weeks sonogram appointment to arrive 15 minutes prior to your scheduled appointment time. Please note that babies and young children are not allowed in the exam room during the sonogram.

Prescriptions
At the time of the visit, your provider will prescribe you a sufficient quantity to last until your next visit. If you are running out of medicine, it is likely because you need to make a follow-up appointment.

Prescription refills will only be handled during office hours. Please contact our office and message will be left for the doctor.

Emergencies
If you are our current patient, there is a physician/midwife on-call 24 hours a day, 7 days a week to help you with an urgent medical problem. If your medical condition is life threatening, please call 911.

If an emergency occurs during office hours, please call our main number to get in touch with a physician/midwife who will return your call as soon possible.
If an emergency occurs after office hours, please call our main number and our afterhours phone service will message our provider on-call. Our provider on-call will return your call as quickly as possible. If your problem is not a medical emergency, please call during office hours.

Circumcision
Please be advised that our providers do NOT perform circumcisions.
We recommend that you plan ahead for this (at approximately 32 weeks).

Transfer Patients
Patients transferring from another practice are required to bring their current medical records to their initial appointment, such as blood results and sonogram reports.
